What this document is

This document is a comprehensive transition toolkit designed to support a smooth, inclusive and well-informed transition from preschool to Reception.

The pack brings together practitioner insight, family voice and child perspective into clear, purposeful formats that Reception teachers can quickly understand and use. Rather than focusing on attainment or readiness checklists, the documents centre on strengths, learning behaviours, interests and supportive strategies — ensuring each child is known as an individual.

It includes multiple components to support different transition conversations, from a one-page child passport to SEND-specific transition information and parent-facing guidance.

This document supports professional judgement and partnership working. It does not replace statutory transition processes or school admission procedures.
